It started with what seemed like an ordinary fever.
Eight-year-old Ethan had come home from school complaining that he felt tired. His forehead was warm, and by bedtime his temperature had climbed to 101°F (38.3°C). His parents assumed he had picked up another seasonal virus. They gave him fluids, encouraged him to rest, and planned to call the pediatrician if he wasn’t better the next day.
But overnight, something changed.
Ethan became unusually sleepy. He said his legs hurt, and he didn’t want to stand. His mother noticed he was breathing a little faster than usual, but she thought it was because of the fever.
The following morning, his temperature had risen again.
Concerned, his parents took him to a nearby urgent care clinic.
At first glance, nothing seemed dramatically wrong. Ethan was awake, answered questions, and didn’t appear to have a severe cough or obvious injury. The medical team checked his temperature, heart rate, blood pressure, and oxygen level. While his fever was concerning, many viral illnesses can present in similar ways.
Then the examining physician asked Ethan one unexpected question.
“Can you pull up your pant leg for me?”
His parents exchanged confused looks.
Ethan slowly rolled up his right pant leg.
The doctor leaned closer.
Near Ethan’s ankle was a small cluster of tiny reddish-purple spots.
His mother hadn’t noticed them before.
“They’re probably bug bites,” she said.
The physician didn’t answer immediately.
Instead, he gently pressed a clear plastic slide against the spots.
They didn’t fade.
His expression changed instantly.
He calmly stepped out of the room and returned moments later with additional staff.
Within minutes, arrangements were being made to transfer Ethan to the pediatric intensive care unit.
His parents were stunned.
“What is happening?” his father asked.
The physician explained that the rash, combined with Ethan’s fever and worsening condition, raised concern for a potentially serious infection that required immediate hospital evaluation.
At the hospital, specialists quickly began additional testing.
Blood samples were collected.
Monitoring equipment was connected.
Intravenous fluids and medications were started while doctors worked to determine the exact cause of Ethan’s illness.
